We have a limited number of memberships available before closing our patient panel and beginning a membership waitlist.

Adult
membership

$259 per month (if paid yearly)
$269 per month (if paid quarterly)
$279 per month (if paid monthly)
Membership fees allow for convenient and direct access to Dr. Moore and her staff with no appointment co-pays.

Pediatric
membership

$50 per visit
With a parent member, no monthly membership fee required for pediatric patients. (No longer able to accept pediatric patients without a parent member.)

Upon enrollment, members pay a one time registration fee of $50/patient or $100 per family.
